Job Description

Job Summary:

Assures the appropriate selection, dosing, administering, and dispensing of medication orders.


  • Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems. Recommends formulary alternatives when appropriate.

  • Compounds and dispenses medications for accurate delivery to patients.

  • Provides drug information and education to medical staff and patients.

  • Communicates with physicians and other clinicians as necessary to facilitate care.

  • Other duties as assigned.

  • Demonstrates knowledge of essential policies, procedures, and regulations, including those pertinent to the formulary, controlled substance accountability, clinical drug QI studies and programs, hospital approved drug and investigational protocols, and ASHP technical guidelines for institutional pharmacy practice.

  • Understands and utilizes sterile techniques (USP 797) necessary to prepare sterile products.

  • Staff certified kinetic service utilizes Aminoglycoside and Vancomycin dosing protocols to evaluate initial dosing, identify levels requiring intervention, and modify subsequent doses.



  • Required Credential(s):

  • Licensed Pharmacist credentialed from the Maryland Board of Pharmacy obtained prior to hire date or job transfer date.

Education and Experience:

  • Bachelor's Level Degree

  • Required: Graduate of an accredited school of pharmacy with a five year college program in pharmacy or a six year PharmD program at date of hire

  • Preferred: One (1) year hospital pharmacy experience at date of hire.
